CBD is legal in South Africa within tightly drawn limits set by the South African Health Products Regulatory Authority (SAHPRA). Government Notice 586 of 23 May 2019, extended in 2020 and continued under subsequent schedule amendments, exempts low-dose cannabidiol preparations from Schedule 4 of the Medicines and Related Substances Act 101 of 1965.

To qualify, products must deliver a maximum daily dose of 20 mg of CBD, contain no more than 600 mg of CBD per pack, contain not more than 0.001 percent THC, and avoid medicinal claims. Such products may be sold over the counter as complementary medicines. Higher-strength CBD products remain Schedule 4 and require a doctor's prescription, while finished pharmaceuticals such as Epidyolex follow the standard SAHPRA registration route. Cosmetic CBD and food supplements are constrained because the Department of Health has not yet finalized hemp-food regulations under the Foodstuffs, Cosmetics and Disinfectants Act 54 of 1972.
