The short answer

CBD is effectively illegal in Vatican City State. The Vatican does not maintain a separate medicines authority for cannabinoid products, and Italian law applied through the concordat framework governs pharmacy and narcotics matters within the city-state.

The Vatican Pharmacy (Farmacia Vaticana), one of the busiest pharmacies in Europe by volume, dispenses medicines registered through the Italian Medicines Agency (AIFA) under Italian law. Following Italian Health Ministry decrees that classified oral CBD as a narcotic in 2020 (subsequently suspended in 2021 by the Lazio Regional Administrative Court but the legal posture remains restrictive for cannabis-derived medicines), CBD products are not freely sold in Vatican retail. Sativex and Epidyolex are available only through specialist prescription under Italian medical cannabis rules. Recreational or wellness CBD products are not authorized for retail sale.
